All Dental Surgeons interested in obtaining PDA (CC) Membership, are requested to download PDA (CC) Membership Form from PDA (CC) website www.pda.org.pk.
Duly filled Membership Forms along with the Membership Fees of PKR 2,000 (For 12 Months) by Cash, Pay Order or Demand Order may be submitted or sent by courier to PDA Headquarters (Address given on Membership Form). PDA (CC) Identity Cards will be dispatched to Members within 30 days of receipt of Membership Form and Membership Fees.Valid PDA CC Members will be entitled to 25% discount on all PDA CC events, besides other privileges announced on PDA CC website from time to time.
